Comments:

An unannounced renewal inspection was conducted between the hours of 9:45am through 11:00am. There were a total of 8 children present (10 points) with the Provider and 1 assistant. A total of 4 children?s files and 2 staff files were reviewed. The children were engaged in the following: playing outside, handwashing, eating morning snack, and participating in various Montessori activities. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, emergency drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There was an abundant supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 5/24/2022. Description:	Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #2 (First Date of Attendance: 2/28/2022) did not have a physical examination on file.
Plan of Correction:	Parent is working on this.
Standard #:	8VAC20-800-60-B
Description:	Based on record review, the provider did not ensure that all children's records contained documentation that each child's parent completed an annual review of the emergency contact information to ensure information is correct. Evidence: Child #3's annual review of the emergency contact information was last competed 2/17/2021 and Child #4's annual review of the emergency contact information was last completed on 11/22/2019.
