22.1 Background Checks Code; carbon monoxide detectors

Comments:
An unannounced monitoring inspection was conducted today from 11:20am-12:17 pm. There were 10 children directly supervised by 3 staff. The indoor and outdoor child care area, 2 staff records, 3 children?s records, 1 household member?s records, evacuation drills, medications, emergency supplies, and policies were inspected. Children were observed eating lunch and playing with manipulatives. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation, written parental authorization for topical skin products that note any adverse reactions were not present for any topical ointment present on the date of inspection.
Evidence:
There were no written parental authorizations observed on the date of inspection. Various topical ointments for children were observed in the family day home on the date of inspection.

Plan of Correction: Written parental authorization for the use/non-use of medications IS on file for each and every child. Moving forward we will request authorization for specific product names from parents for each agent.

Standard #: 8VAC20-800-900-A
Description: Based on observation, the family day home did not assist to prevent choking by serving foods considered to be choking hazards that was not prepared before serving in a manner that will reduce the risk of choking.
Evidence:
On the date of inspection, the lunch served to some of the children under the age of 4 consisted of whole hotdogs that were not cut lengthwise to prevent choking.

Plan of Correction: Hot dogs will be cut lengthwise moving forward.

Standard #: 8VAC20-800-920
Description: Based on documentation and observation, the posted menu did not list substituted food.
Evidence:
On the date of inspection, the posted menu listed: Lentils, brown rice, bell peppers, broccoli, and milk for lunch. The lunch that was served consisted of: Hotdog, bun, cucumbers, and milk.

Plan of Correction: The Tuesday and Wednesday lunch menus were simply switched. Hotdogs, buns. cucumbers, and milk are regular items on the weekly menu.

Standard #: 8VAC20-800-60-B
Description: Based on record review, not all children?s record contained all required information.
Evidence:
1. Child 1?s record did not contain the child?s health insurance policy number.
2. Child 1, 2, 3?s record did not contain a current and up-to-date written authorization for the caregiver to administer medications.

Plan of Correction: 1. The said child's health insurance number will be added. 2. The provider is currently using the last option on the Medication Administration Form, which excludes the use of EpiPens. Instead, the provider will use option 3 on the same form, which includes EpiPens along with non-prescription topical agents.

Standard #: 8VAC20-800-320-G
Description: Based on record review, wood burning fireplaces and associated chimneys were not inspected annually by a knowledgeable inspector to verify the devices are properly installed, maintained and cleaned as needed.
Evidence:
The fireplace and chimney were last inspected on 4/2/2021.

Plan of Correction: It has been difficult obtaining an appointment for the inspection of this chimney; however it will be done as soon as possible. For the record, this chimney has never been in use.

Standard #: 8VAC20-800-540-B
Description: Based on observation and provider statement, infant walkers were used by the family day home.
Evidence:
An infant walker was observed at the family day home on the date of inspection. The provider stated that it is used by at least one child enrolled in the program.

Plan of Correction: The provider is fully aware of the policy regarding infant walkers. The "walker" in question was physical aid used by a child with a certain medical condition. It will no longer be used.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not non absorbent.
Evidence:
The diaper changing pad was covered with an absorbent fabric cover.

Plan of Correction: In consideration for infant safety and hygiene, the provider uses disposable waterproof liners for each diaper change. The practice also helps avoid the use of harmful chemicals in disinfectant sprays. Moving forward, we will not be using liners and instead plastic -covered changing pad.
