54.1 Provider must be MAT certified to administer prescription medication.

Comments:
A monitoring inspection was initiated on 5/20/2022 and concluded on 5/20/2022. There were a total of 9 children (21 points) observed with 2 caregivers present. The children were engaged in the following: toy play, manipulatives, breakfast, handwashing, and bathroom breaks. A total of 1 new child record was reviewed. The caregiver/household records are current. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-830-B
Description: Based on record review and provider interview, Shelter-in-place procedures are not practiced a minimum of twice per year.
Evidence: The emergency evacuation form was reviewed, there was no documentation of 2 shelter in place drills. The last documented shelter in place drill was dated 4/9/2021.
The provider said they have not completed a shelter drill in over a year.

Plan of Correction: We will conduct shelter drills 2 times a year. We will do one in May and one in June.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas and furnishings of the family day home, inside and outside, have not been maintained in a clean, safe, and operable condition.
Evidence: There was a piece of wood observed on the outside step railing that is loose, it could cause a finger entrapment injury.

Plan of Correction: We will secure the piece of wood to the railing.

Standard #: 8VAC20-800-430-A
Description: Based on observation and provider interview, children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision have access to stairs.
Evidence: There was a gate at the top of the outside stairs but it was not secured. There was no way to secure the gate, allowing children to freely have access to the outside concrete stairs.
The provider stated that she needs to attach the gate to the side of the house so the children cant access the stairs.

Plan of Correction: We will find a way to lock the gate or we will replace the gate with a new one.
