Technical Assistance:
Discussed with the provider the importance of updating FDH?s polices when changes occur so the policy reflects these changes: The provider is not currently MAT trained so is not authorized to administer medication., The FDH policy on medication administration should reflect that the provider will not administer medication to children under her care. Parents shall be made aware of these changes and signed acknowledgements be included in each child?s record.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-11:37am. There were 12 children (30points) directly supervised by 3 caregivers. The home, 2 staff records, 2 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed while playing with Legos, dancing, singing and eating snack. Caregivers were actively supervising and engaged in conversations with the children.

Violations:
Standard #: 8VAC20-800-760-A
Description: Based on review of emergency supplies, the first aid kit did not contain all required components.
Evidence:
On the date of inspection, the First aid kit did not contain a second triangular bandage.

Plan of Correction: I asked the family to bring it back.

Standard #: 8VAC20-800-830-A
Description: Based on documentation review, emergency evacuation procedures were not practiced monthly.
Evidence:
On the date of inspection, the 2021 emergency evacuation drills log provided for review did not have an evacuation drill documented for the month of October 2021.

Plan of Correction: We did practice the emergency evacuation but forgot to write it on the paper. (10/14/2021)

Standard #: 8VAC20-800-50-A
Description: Based on review of the written attendance record and interview with the provider, the provider did not ensure all children were included on the written attendance record.
Evidence:
1) The written attendance record showed 5 students in care, however there were 12 students present in the family day home. 2) The provider stated that she uses the infants daily notes as part of keeping track of the attendance.

Plan of Correction: Remind to the families to sign in and out everyday.

Standard #: 8VAC20-800-70-A
Description: Based on documentation review, parents were not provided in writing of all required information before the child?s first day of attendance.
Evidence:
The record for Child #1 did not contain a signed acknowledgement of having received the fdh?s liability insurance declaration form. The record for Child # 2 did not contain a signed acknowledgement of having received the fdh?s administration of medications policy.

Plan of Correction: Parent will sign the form.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially hazardous substances such as cleaning agents were stored in areas that were not inaccessible to children.

Evidence:
Sanitizing wipes were stored in an unlocked cabinet on a low shelf next to the diaper changing table where children can have access. A spray bottle with a cleaning solution was observed on the bottom staircase leading into the family day home. The bottle was not locked and children could have had access to the bottle.

Plan of Correction: Children never open the door. But it will be relocated.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence:
On the wall to the right of the entrance to the infant room there was an outlet missing a protective cover. Ages of children in care on the date of inspection were between 4 months -5 years.

Plan of Correction: The only one electrical outlet that was not covered will be covered now.
