Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training.

A blank copy of the Administration of Medication policy form was emailed to the FDH, along with the inspection reports.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 06/02/2022, between the hours of 10:30 am to 12 pm. There were 2 children present (6 points), ranging from infant to preschool-age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records,2 household records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there were (2) two non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 5 children files indicated that the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached was not available.

Evidence -
1. On the date of inspection, the emergency contact information consisting of two individuals, were not available for: Child #4, and Child #5.
2. Both Child #4, and Child #5 had only 1 of the 2 required contacts listed.

Plan of Correction: The Parents of Child #4, and Child #5 will be contacted and an additional emergency contact will be added to each of their files.

Standard #: 8VAC20-800-70-A
Description: Based on review, 5 of 5 children files indicated that not all of the required information was provided in writing.

Evidence -
1. On the date of inspection (06/02/2022), the Policies for the Administration of Medications form was not available for review for: Child #1, Child #2, Child #3, Child #4, and Child #5.

Plan of Correction: A blank copy will be completed for each child in care and placed in their file.
