Comments:

A monitoring inspection was conducted on 6/9/22 from 11:00am until 12:00pm. At the time of entrance, there were 4 children in care, for a total of 8 points, with the provider. A sample of 4 children's records and 1 provider records were reviewed. Children were observed engaging in free play, watching an education program on television and transitioning to nap time. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and family day home postings were reviewed. The provider stated there have been no accidents or injuries to children in care since the last inspection. Information gathered during the inspection determined non-compliance with applicable standards or law. One violation is listed on the violation notice and was discussed with the provider during the exit interview.

Description:	Based on record review and interview, the provider did not ensure that children's records contain all the required information. Evidence: 1. The record for child 1 (date of enrollment: 4/25/22) does not contain documentation that the provider obtained verification of the identity and age of the child. 2. The provider confirmed that the record for child 1 is lacking documentation that the child's age and identity was verified.
Plan of Correction:	The provider responded with the following: Child 1's birth certificate was viewed by the provider prior to enrollment; however, the provider did not document that the birth certificate was viewed. The provider will contact the parent and ask them to bring in the birth certificate in order to document it in the child's record.
