Comments:
An in-person monitoring inspection was conducted on June 1 2022 from 11:45am to 12:30pm. There were 5 children present with 1 staff supervising. The children were observed eating lunch, washing hands and getting ready for nap time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 1 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: Child 2 does not have documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. (Date of enrollment 4/4/20)

Plan of Correction: I will get the physical from the child's parents.

Standard #: 8VAC20-800-60-B
Description: Based on record review, all required information was not in the child's record.

Evidence: Child 2 does not have 2 addresses for emergency contacts.

Plan of Correction: I will have the parents give the information.

Standard #: 8VAC20-800-80-A
Description: Based on record review, proof of identity was not obtained within 7 days.

Evidence: Child 2 does not have signed documentation of viewing the proof of identity for the child.

Plan of Correction: I will get the proof of identity.

Standard #: 8VAC20-800-90-C
Description: Based on record review, updated immunization records were not obtained for each child.

Evidence: Child 1 has vaccination records dated 10/2020. (Date of enrollment, 8/1/2020)

Plan of Correction: I will get the records.

Standard #: 8VAC20-800-240-A
Description: Based on observation, furnishings in the home were not maintained in a safe condition.

Evidence: A white crib was observed to be held together at the corners with duct tape. A blue crib was observed to be fastened together using zip ties.

Plan of Correction: I will replace the cribs.
