Technical Assistance:
Provided consultation on:
1) ensuring that there are extra batteries on hand for the battery operated weather band radio
2) ensuring that parents complete all information on the registration form or use N/A
3) refer to the standards for a complete list of required items in the first aid kit; more than one triangular bandage is required
4) document all caregiver training in records
5) document submission of background check requests in caregiver records; follow up required if not received within time frame

Comments:
An unannounced monitoring inspection was conducted from 10:15 - 11:30 am with the provider. There was one child in care, age 14 months, for a total of four points. The child was observed napping, waking, having a diaper change and having free play with a variety of age-appropriate toys. One child record, two caregiver records, written attendance, weekly menu and the first aid kit were reviewed. Required information was posted. The last emergency escape drill was conducted: 5/12/22 and the last shelter-in-place drill was conducted: 4/6/22. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Description: Based on review of two caregiver records, it was determined that not all caregivers had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check prior to their first day of employment. Evidence: the record for caregiver #2 (date of hire: 1/4/2022) did not have an OBI determination letter.

Description: Based on observation, and an interview with the provider, it was determined that the home did not have a carbon monoxide detector as required. Evidence: the provider stated that the home does not have a carbon monoxide detector.

Description: Based on observation, and an interview with the provider, it was determined that not all medication was kept in a locked place, using a safe locking method that prevents access by children. Evidence: three medications were observed inside an open grocery-type bag sitting on the floor.
