A subsidy health and safety (SHSI) inspection was conducted on June 15, 2022 from 11:45 A.M.-1:30 P.M. There were two children present with one caregiver supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-250-F Description: Based on review of caregiver records, the provider failed to ensure the annual health and safety training was updated yearly.
Evidence:
1. The last documented Health and Safety update training on file was dated 02/06/2020. There was not an updated training on file for 2021.
2. The provider verified that the training had not been completed since 2020.Plan of Correction: The provider will complete the training as soon as possible.
