Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on June 14, 2022 from 2:42 p.m. through 3:51 p.m. There were ten children in the care of the provider and one assistant for a total of eighteen points. Three children's records and the records for the provider, assistant and one household member were reviewed/updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Interactions between the provider and children were observed as positive and the children seemed comfortable with their daily routine and schedule.

Standard #: 8VAC20-800-150-D-1 Description: Based on record review and discussion, the provider failed to ensure that assistant maintained a current certification in cardiopulmonary resuscitation (CPR).
Evidence:
1. Assistant Provider's cardiopulmonary resuscitation (CPR) certification expired on 11/2021.
2. Provider confirmed assistant's CPR certification is expired.Plan of Correction: I have her scheduled to take the class in a couple weekends.
Standard #: 8VAC20-800-150-D-2 Description: Based on record review and discussion, the provider failed to ensure that assistant maintained a current certification in first aid.

1. Assistant Provider's first aid certification expired on 11/2021.
2. Provider confirmed that the assistant's first aid certification expired.Plan of Correction: I have her signed up to take a class in a couple weekends.
Standard #: 8VAC20-800-180-A Description: Based on a review of caregiver records, it was determined the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening.
Evidence:
1. Provider's tuberculosis screening expired on 06/09/2022.
2. The provider confirmed the TB screening was expired.Plan of Correction: I will schedule an appointment to get an updated TB screening next week with my doctor.
