Technical Assistance:
Discussion and observation of available space both inside the FDH and the outside back play area. The Contact with the Local Zoning Administrator has approved a total capacity for no more than 7 children at any one time. The new location is still in a state of flux. Once new location is approved, all remaining files, equipment, and care provided will be transferred to 3212 Gary Court.

Discussion also centered around new household member and what documentation will be required (FPC, CPS, SDS, and TB). I would encourage to contact OBI to inquire about the portability process for the existing background checks from Virginia. All background checks must reflect the address of the new location of 3212 Gary Court.

In addition an identification card was submitted from the State of Maryland with an issue date of 07/22/2020 indicating prior residence within the last 5 years. Therefore, in addition, out-of-state CPS check from Maryland and a Sex Offender Check from Maryland will also be required for Household Member #1.

Discussion of also needing to review CPS check for new assistant that was sent back in March. This documentation was still at the old address. Provider will make this documentation available for review. New home offers plenty of space both inside and out for the children to enjoy. All undetermined areas of this initial visit will be reviewed at the next unannounced mandated inspection, to include subsidy regulations.

Comments:
This initial inspection was initiated by licensing staff and concluded on 06/21/2022, between the hours of 10 am to 11 pm. There were no children in care. This inspection was needed in order to perform a modification to the current license. This modification will be a change of address to reflect the new address as: 3212 Gary Court, Falls Church, Virginia 22042.. The inspector reviewed compliance in the areas of administration; physical plant; special care and emergencies. A total of 8 children records were previously reviewed during an UMI that took place on 03/31/2022. During that time both the Provider's and Assistant's records were reviewed as well.

The information gathered during the inspection determined that there was not a violation to an applicable standard, code, and/or law. Therefore, a Violation Notice was not issued.

If you should have any questions, please call or send an email to charlie.perkins@doe.virginia.gov. Thank you.

Charlie Perkins,
Licensing Inspector
