Comments:
An unannounced monitoring inspection was conducted on Wednesday, June 8, 2022 from approximately 12:25 PM to 02:00 PM to determine the provider's compliance with licensing standards. In attendance were six non-resident children. The children were being supervised by two caregivers during the inspection. There was a total of eighteen points based on the children?s ages. The children were observed sleeping and watching children?s programs on television. A walk through of the lower level of the home was conducted. The licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, required postings, menu, caregivers training hours and daily attendance. The provider stated medications are not being administered at this time. Transportation is not provided. During the inspection five children?s and two caregivers? records were reviewed. The provider was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Description: Based on observation, medications were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A bottle of Allergy medicine was unlocked on the kitchen counter accessible to children.

Standard #: 8VAC20-800-100-A
Description: Based on review of children's records, the provider did not ensure to obtain a physical examination two of five children. Evidence: 1. Child #3 did not have a physical examination on file. The child was enrolled on 4/25/2022. Child # 5 was enrolled 11/22/20 and the physical was received 1/21/22.

Standard #: 8VAC20-800-50-A
Description: Based on observation and interview it was determined that the provider failed to maintain a written record of attendance daily. Evidence: 1. The provider was unable to provide sign in and out sheets. It was reported through interview that the provider uses her USDA forms to keep track of children in attendance and she had just turned in her forms to USDA.

Description: Based on a review of five records for children, and an interview with the provider, the immunization records for two children were not obtained before initial attendance. Evidence 1. Child 3 begin attending on April 4, 2022, and there was not an immunization report signed by the health care provider in the file. Child 5 began attending 11/22/02021 and immunization was received 1/21/22.

Description: Based on observation and interview, the applicant did not ensure sharp objects were inaccessible to children unless being used by the caregiver or with children under close supervision. Evidence: 1. There were 2 sharp knives located in an unsecured drawer in the kitchen.

Description: Based on observation, the provider did not ensure that children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.
