Technical Assistance:
The licensing inspector and provider discussed shelter-in-place drills need to be conducted twice a year. Emergency evacuation practice drills need to be conducted monthly. Provider discussed MAT training.

Comments:
An unannounced renewal inspection was conducted with the provider. There were three children, ages 19 months, 2, and 4 years old, with one residential child in the care of the provider for a total of 11 points. The children were observed playing and eating lunch which included tuna sandwiches, squash, apple slices, and milk. Three children?s records were reviewed. The provider and one household member?s records were reviewed and in compliance. There were no medications on site to be reviewed. The most recent emergency evacuation drill was documented as February 18, 2022. The first aid kit, flashlight, and battery-operated radio were observed and in compliance.

Standard #: 8VAC20-800-830-A
Description: Based on a review of documentation, the provider did not conduct emergency evacuation practice drills every month as required. Evidence: The most recent emergency evacuation drill was documented as February 18, 2022. There was no documentation for an emergency evacuation drill for the months of March, April, May of 2022.

Plan of Correction: The provider will conducted monthly emergency evacuation drills and ensure that they are recorded on the log.

Standard #: 8VAC20-800-100-A
Description: Based on a review of documentation, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child # 1, start date 3/28/22, did not have documentation of a physical in their record.

Plan of Correction: The provider will obtain the missing documentation from the child's parent and place it in their record.

Standard #: 8VAC20-800-90-A
Description: Based on a review of documentation, the provider did not obtain documentation of that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: Child # 2, start date 2/8/22 did not have documentation of an immunization in their record.

Plan of Correction: The provider will obtain the missing documentation from the parent and place it in the child's record.
