Technical Assistance:
1. Discussed provider's interest in moving the daycare to the lower level of the home.
2. Reviewed menu requirements. Inspector will share model form.
3. Discussed the importance of crib sheets be pulled tightly to the mattress before the child occupies the crib.
4. Reviewed the current medication policy, and explained that the policy needs to match what is written on the medical policy form that parents sign.
5. Reviewed the provider's responsibility to review each child's proof of identity, and to document in the enrollment record.
6. Recommended vacuuming occur prior to children arriving daily to ensure the play areas are clean and free of debris and small particles.

Comments:
An unannounced monitoring inspection was conducted at 10:45am. Observed the provider with 1 child = 4 points. Ratios and supervision were in compliance. The child was playing with toys and in an excersaucer. The daycare areas of the home were sufficiently supplied with toys and equipment for the children. The provider reported that lunch today would include: sweet potato baby food (in a jar), rice, beans, milk, and water. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Violations:
Standard #: 8VAC20-800-920
Description: Based on interview conducted, it was determined that the provider has not been writing a weekly menu of food served. Evidence: The provider said she has not been writing a menu, but does complete the USDA menu reports. There was no menu of food served for the previous or current one-week periods.

Plan of Correction: The provider will plan and write a menu for each week, and keep menus on-file for at least 1 week. The menus will be dated and made available to parents.

Standard #: 8VAC20-800-60-B
Description: Based on 1 child's record reviewed, it was determined that required information was not documented. Evidence: Child #1 - The information for parents form was not signed by the parent, the medication formed signed by the parent had an inaccurate policy, 2 of 2 emergency contacts were not completed, there was no documentation of medical insurance and policy number, and no documentation of the provider viewing the child's proof of identity.

Plan of Correction: The inspector reviewed forms with the provider who will discuss with parents and complete as required.

Standard #: 8VAC20-800-690-D
Description: Based on observations made, it was determined that the diapering surface was not cleaned and sanitized after each use. Evidence: After changing a diaper the provider did not clean or sanitize the surface. When prompted by the inspector, the provider sprayed a disinfectant cleaner on the surface, but did not clean it first with soap + water. The provider said she thought it was only necessary when the surface was shared by multiple children.

Plan of Correction: The provider will clean the diaper changing surface with soap + water, and then spray it with a sanitizing solution and let it air dry between every diaper change.

Standard #: 8VAC20-820-120-E-2
Description: Based on observations made, it was determined that the findings of the most recent inspection were not posted in the home. Evidence: The provider said she had not posted them because she is shopping for a bulletin board.

Plan of Correction: The provider will find a suitable place near the front door of the home to post inspection findings.
