A monitoring inspection was completed from 1:50 p.m. until 3:00 by two inspectors. There were 12 children present, ranging in ages from infant 5 yrs. old for a total of 30 pts. with two providers and one household member present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and four 4 provider and/or household member records were reviewed. Children were observed resting and being fed a bottle.

The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-140-D
Description: Based on interview, the substitute provider failed to maintain a record of arrivals and departures on each day that the substitute provider works.

Plan of Correction: The substitute provider will start to document hours.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider failed to keep poisonous substance supplies inaccessible to children.
Evidence:
1. the inspector observed a bottle of hydrogen peroxide, two bottles of oral rinse and other personal care items stored under the bathroom sink that the children use.

Plan of Correction: Corrected during the inspection.

Description: Based on observation, the provider failed to keep an electrical device unplugged by the bathroom sink.
Evidence:
1. The inspector observed a flat iron unplugged near the bathroom sink basin that the children use.
