Comments:
A monitoring inspection was conducted on 6/24/22 from 10:45am until 11:50am. At the time of entrance, there were 5 children in care, for a total of 9 points, with the provider. A sample of 5 children's records and 2 provider/household member/assistant records were reviewed. Children were observed engaging in free play, singing songs and preparing for lunch. Restroom and handwashing procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and family day home postings were reviewed. The provider stated she does not currently administer medication to children in care. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are listed on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on record review and interview, the provider did not ensure that children's records contain all the required information.

Evidence:
1. The record for child 1 (date of enrollment: 9/8/21) does not contain the addresses of two designated persons to contact in case of an emergency if the parent cannot be reached.
2. The provider confirmed that the record for child 1 is lacking the addresses for the two emergency contacts.

Plan of Correction: The provider responded with the following: The parent of child 1 was contacted during the inspection to request addressed for the two emergency contacts.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interview, the provider did not ensure that a current Report of Tuberculosis Screening form is obtained from each caregiver every two years from the date of the first screening.

Evidence:
1. The most recent tuberculosis screening for caregiver 1 is dated 12/3/19. Caregiver 1 was due for an updated TB screening in December 2021.
2. Caregiver 1 confirmed that her TB screening is overdue.

Plan of Correction: The provider responded with the following: This was an oversight. Caregiver 1 will obtain an updated TB screening as soon as possible.
