Comments:

An unannounced monitoring inspection was conducted on June 21, 2022 from approximately 9:50 am to 10:55 am. There were 5 children in attendance. The provider record, substitute provider record, assistant records and all household member records, along with 5 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. Interviews were held with the provider and caregivers throughout the inspection. The inspector interacted with children in care when appropriate. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys to meet the needs of the children. Children were observed playing with playdough, playing with toy cars, and playing with slime. The license and current inspection were posted as well as the drill log. Evacuation drills were conducted monthly. The most recent was on 06/17/22. Shelter in place drills were conducted two times with the most recent on 04/08/22. The emergency preparedness plan was reviewed and updated annually. The menu was posted. Per the provider, medication is not administered, and transportation is not provided. The center?s first aid kit was inspected and found complete. The home had a working flashlight and battery operated radio. There were violations cited as a result of this inspection. Standard #:	22.1-289.035-A
Description:	Based on review of records and interview, the provider did not ensure that the background checks for one of two employees were updated every five years. Evidence: Staff #3's last Central Registry was done on 02-22-17 and was due to be repeated no later than 02-22-22. When interviewed the provider stated that she had mailed the request on 05-16-22 but never received the results.
Plan of Correction:	I will not let staff #3 work until they get this done.
Standard #:	22.1-289.036-A
Description:	Based on review of records and interview, the provider did not ensure that the background checks for one of two licensees/household members were updated every five years. Evidence: 1) Staff #1's last Central Registry was done on 11-2-16 and was due to be repeated no later than 11-2-21. When interviewed the provider stated that she had mailed the request on 05-16- 22 but never received the results.
Plan of Correction:	I will call them and check on the results.
Standard #:	8VAC20-800-100-A
Description:	Based on a review of records, the provider did not obtain documentation of a physical examination within 30 days of a child?s attendance for one of five children. Evidence: Child #5?s file, date of enrollment 11-15-21, did not contain a physical.
Plan of Correction:	I will ask the parent to get it to me.
Standard #:	8VAC20-800-180-A
Description:	Based on a review of records, and interview, the provider did not ensure that each caregiver obtains an updated TB every two years. Evidence: 1) Staff #2's last TB was done on 04/01/20. The updated test results should have been completed on or before 04/01/2022. 2) Staff #3's last TB was done on 04-24-19. The updated test results should have been completed on or before 04-24-21. When interviewed, the provider stated that she was unaware that staff #3 was due for updates.
