Violations:
Standard #: 8VAC20-800-40-A
Description: Based on observation and interview, the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: At 12:15 pm two children got off the bus and came into the home increasing the number of children in care from 11 to 13. The home is only licensed to care for 12 children. When asked, the provider stated that the children are usually picked up by their aunt at the end of the driveway but do sometimes have to come in and wait for a few minutes. The children were picked up at 12:16 pm.

Plan of Correction: Per the Provider "I will not allow more than 12 children in one day."

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider did not ensure that areas and furnishings were maintained in a clean, safe, and operable condition. Evidence: 6 out of 11 children?s rest mats were ripped and had exposed foam. The children were observed sleeping on the mats.

Plan of Correction: Per the Provider "I will purchase new mats ASAP."
