An unannounced monitoring inspection was conducted today between 3:40pm and 4:45pm. There were 6 children present (10 points) with the Provider within the supervision guidelines. The children were observed engaging in various activities and playing with age appropriate toys. Positive interactions were observed during this inspection. A complete inspection of the physical plant, outdoor play area, 3 children's records, 1 caregiver record, 1 household member record, evacuation drills, attendance, emergency materials, were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-800-100-A Description: Based on a review of children's records, 1 out of 3 records reviewed did not contain documentation of a physical examination within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment: 4/26/2021) did not have documentation of a physical examination on file at the time of the inspection.Plan of Correction: Provider contacted the parent the same day of the inspection to ask them to bring a copy of the child's physical examination the following day.
