Comments:
A monitoring inspection was conducted on 06/23/2022 from 11:42 a.m. ? 3:15 p.m. There were
7 children present during the inspection and two providers present. The inspector reviewed
compliance in the areas of administration, qualifications and training, physical plant, staffing
and supervision, programming, medication, special care and emergencies, and nutrition. Five
(5) children?s record, 2 provider record were reviewed. The information gathered during the
inspection determined violations with applicable standards. A violation notice was issued.

Violations:
Standard #: 8VAC20-800-830-C
Description: Based on review of emergency response drill documentation, the provider failed to ensure the
documentation included all of the required information.
Evidence:
The emergency drill documentation dated January 2022 ? June 2022 did not document: the
method used for notification of the drill; the number of caregivers participating;
the time it took to complete the drill; participating; any special conditions simulated;
problems encountered, if any; and weather conditions for evacuation drills only.

Plan of Correction: Take action 06/23/2022

Standard #: 8VAC20-800-60-B
Description: Based on a review of children?s records, the provider failed to ensure that two of five children?s
records contained all of the required information.
Evidence:
1. The records for child, C2 and child, C3 did not document the children?s date of
attendance (DOA).
2. The records for C2 and C3 did not contain the results of the health examination for the child as required by 8VAC20-800-
100.
3. The record for C2 did not contain the parental signed acknowledgement of the receipt
of the information required by 8VAC20-800-70.
4. The record for C2 did not contain a written authorization for emergency medical care
should an emergency occur and the parent cannot be located immediately unless the
parent presents a written objection to provision of medical treatment on religious or
other grounds.

Plan of Correction: Take an action as of June 23. Sitting down with mother going through all records.

Standard #: 8VAC20-800-430-A
Description: Based on observation, the provider failed to ensure that a child under two years
of age did not have access to stairs.
Evidence:
On 06/23/2022, the inspector observed the installed baby gate used to prevent a child?s access
to stairs was not being used. The gate was partially wedged behind a piece of furniture and a
child under the age of two was present in the home.

Plan of Correction: The gate is always used. The gate is out and in use.

Standard #: 8VAC20-800-520-A
Description: Based on observation and interview, the provider failed to provide a crib for children from birth
through 12 months of age.
Evidence:
The provider reported disposing of a crib due to peeling paint; however, had not set up a new
crib for the child in the home that provider reported being 11 months old.

Plan of Correction: The crib already there and it there put up the same days.
