Information and support contact numbers were given to the provider on obtaining both in state and out of state background checks. Missing background checks are a high risk violation and will prevent a new license from being issued if they are not obtained prior to expiration (September 2022)

Comments:
An unannounced inspection was conducted on-site. The provider was watching three children during the inspection for eight points with two caregivers which is within ratios for supervision. Reviewed physical space, emergency preparation, transportation, nutrition, infant care and records. Background checks must be completed in a timely manner to ensue the new license will be issued in September. The provider was given extra time to locate missing training documentation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on observation, the Provider failed to renew her CPS check prior to expiration.
Evidence:
The Provider's CPS clearance check expired 12/2021 and a new one has not been obtained prior to expiration.

Description: Based on record review and provider statement, an out of state Central Registry check has not been obtained within 45 days of employment.
Evidence:
The assistant lives out of state within the past 5 years and does not have a CPS check-hired September 2021.

Plan of Correction: The InterState Background chart was shared to the provider to assist in obtaining the out of state CPS check on June 23, 2022.

Description: Based on record review and statement, a caregiver is missing a Virginia Central Registry Check.
Evidence:
Assistant is missing required VA CPS check within 30 days of employment. Hired September 2021.

Plan of Correction: The Licensing Inspector sent information June 23, 2022 on obtaining the Virginia CPS checks through the CCAPS portal.

Description: Based on provider statement, the provider did not ensure that all required items were in the while transporting children.
Evidence:
The provider stated no first aid kit and provider information (provider's address and phone number, written permission to transport as well as emergency information for children) were in the car.

Plan of Correction: Supplies and Information will be placed in the car prior to providing transportation.

Standard #: 8VAC20-800-70-A
Description: Based on record review, child #1 parent did not have all required information prior to the child attending the Family Day Home.
Evidence:
1. Child #1 was missing a signed acknowledgement of the home's medication policy, liability declaration, provision of emergency relocation, transportation permission and authorization to apply topical creams.
2. Child #2 was missing the transportation permission form.

Plan of Correction: The parent will be provided all missing forms, obtain signature and will file in the child's record.

Standard #: 8VAC20-800-320-B
Description: Based on observation, unused outlets covers were missing protective covers.
Evidence:
Two outlets were missing covers in the daycare space in the basement.

Plan of Correction: Protective covers will be placed in all unused outlets in the daycare space.

Standard #: 8VAC20-800-590-A
Description: Based on observation an infant is not placed on their back when sleeping.
Evidence:
During the inspection an infant fell sleep and was placed on their stomach on the provider's lap.

Plan of Correction: When the baby falls asleep they will be placed on their backs in a crib.
