Comments:
An unannounced renewal inspection was conducted on 7/15/22. There were 4 child care children and 3 of the provider's own children (13 points) directly supervised by a caregiver. The home, 1 staff record, 4 children's records, 1 household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Description: Based on review, the provider did not ensure that documentation of physical examinations were obtained prior to the child's attendance or within 30 days after the first day of attendance.
Evidence: reviewed four children's records and found that Child A did not have documentation of a physical examination.
