Technical Assistance:
Technical assistance was provided in the following areas: Carbon monoxide detectors, child records, caregiver records, annual training, fireplace inspections and hazardous substances.

Comments:
An unannounced renewal inspection was conducted on 7/18/22 from 5:00pm - 6:30pm. At the time of the inspection there were 5 children in care, ages six months old to five years old (7 points) with two caregivers present. Records were reviewed for five children and two caregivers/household members. Children were observed participating in various activities. There was no medication at the family day home currently. Emergency plan and emergency supplies were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed with the provider during the exit interview.

Violations:
Standard #: 8VAC20-800-760-A-1
Description: Based on a review of the family day home's first aid kit, it was determined that the provider did not ensure that the first aid kit contains all of the required items.

Evidence:
1. The first aid kit for the family day home did not include any triangle bandages.
2. Caregiver #1 confirmed that the first aid kit did not include any triangle bandages.

Plan of Correction: The provider responded: We used the triangle bandage that was in the first aid kit, but I we will purchase some additional triangle bandages.

Standard #: 8VAC20-800-800-B
Description: Based on a review of the emergency plan and interview, it was determined that the provider did not ensure that a review the emergency plan shall occur at least annually and update the
plan as needed. The provider shall document in writing each review and update to
the emergency plan.

Evidence:
1. The emergency plan had not be reviewed in more than a year.
2. Caregiver #1 confirmed that the emergency plan had not be reviewed in more than a year.

Plan of Correction: The provider responded: I will review the emergency plan and make sure it is reviewed annually.

Standard #: 8VAC20-800-70-B
Description: Based on a review of five child records, it was determined that the provider did not ensure that each child's record contain documentation of the parent's written acknowledgement of the disclosure of the information for parents.

Evidence:
1. None of the five child records reviewed contained documentation of the parent's written acknowledgement of the receipt of the information for parents that is to be disclosed prior to the first day of attendance..
2. Caregiver #1 confirmed that none of the child records contained the parent's written acknowledgement of the receipt of the information for parents

Plan of Correction: The provider responded: The documentation had been obtained previously, but I was unable to find it during the inspection. The parents of the children will be given a new copy of the information for parents and all supporting documents to sign.
