Comments:
An unannounced monitoring inspection was initiated and concluded on 7/25/2022. The inspector was on site from 12:10 pm-1:15 pm. There were 7 children in care ranging in age from 3-9 years with 2 caregivers. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 child records, and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-800-780-B
Description: Based on a review of 5 children's records and interview, the provider did not ensure that 1 record contained the parent's signed acknowledgement of an annual review of the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information was correct.

Evidence:
1. The record of child #2 (DOE:06/26/2021) did not contain documentation of the parent's annual review.
2. The provider acknowledged that it was missing.

Standard #: 8VAC20-800-280
Description: Based on observation and interview, the provider did not ensure that potentially poisonous materials and supplies were stored in areas inaccessible to children.

Evidence:
1. A can of Hot Shot insect killer was observed on the floor in a room where a child was napping.
2. The provider acknowledged that the insecticide was accessible to the children.
