Technical Assistance:
Technical assistance was given about batteries and staff documentation.

Comments:
An unannounced monitoring inspection was conducted on 7/18/22 from 10:11am-1:20pm. There were 7 children (19 points) supervised by 3 direct caregivers. The home, 3 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely, Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-770-40-D-4-a
Description: Based on interview and record review, a central registry check for a caregiver was not completed by the end of 30 days of employment.

Evidence: Caregiver #3 did not have a central registry check completed by the end of 30 days of employment. The Provider stated that she emailed the central registry on 3/26/22, but have not received the results. Date of employment for Caregiver #3 is 4/4/22.

Plan of Correction: I will try to find the background check and put in file by 7/29/22

Standard #: 8VAC20-800-780-B
Description: Based on record review and interview, the provider did not review with the parent the emergency contact information to ensure the information is correct and did not obtain the parent?s signed acknowledgment of the review, annually.

Evidence: Child #1 emergency contact information was last reviewed on 3/14/2021

Plan of Correction: I will have annual update on 7/18/22.

Standard #: 8VAC20-800-800-B
Description: Based on record review and interview, the provider did not review the emergency plan at least annually and update the plan as needed.

Evidence: The emergency plan was last reviewed on 3/6/2017.

Plan of Correction: I will review and update by 7/21/22

Standard #: 8VAC20-800-90-A
Description: Based on record review and interview, the provider did not obtain documentation that the child has been adequately immunized before attended the family day home.

Evidence: Child #4 did not have documentation of immunizations before attending the family day home. Child?s first day of attendance was 7/18/22.

Plan of Correction: I will have immunizations on 7/19/22.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas of the family day home located outside was not maintained in a safe condition.

Evidence: There were two wooden logs bordering the playground equipment that were deteriorating and splintered.

Plan of Correction: Logs will be replaced by 7/20/22.
