- Comments:

A subsidy health and safety inspection (SHSI) was conducted today to review supplemental health and safety requirements not covered in the licensing regulations. A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. Areas of non-compliance of the supplemental SHSI requirements were found. Standard #: 8VAC20-790-180-B Description: Based on records reviewed, it was determined that a written record of daily attendance documenting the arrival + departure of each child as it occurs has not been maintained. Evidence: The daily attendance record did not include the times of arrival + departure. Plan of Correction: Implement attendance doc provided by inspector.
Standard #: 8VAC20-790-250-B Description: Based on records reviewed and interview conducted, it was determined that 2 of 2 assistant caregivers did not complete the Virginia Preservice Training for Child Care Staff class (10 hours) within the first 90-days of employment.
Evidence:
1. Assistant #1 ? There was no documentation of completion. Date of employment was 4/1/22.
2. Assistant #2 ? There was no documentation of completion. Date of employment was April 2021.
3. Assistant #2, in the presence of the provider reported that the 10-hour training class had not been completed for either assistant.Plan of Correction: Intend to cancel subsidy effective 7/30/22. Will send request
Standard #: 8VAC20-790-250-F Description: Based on records reviewed and interview conducted, it was determined that the provider did not complete the annual Virginia Health + Safety Update class (3 hours) within 12 months of completion of the Virginia Preservice 10-hour course.
Evidence:
1. The provider completed the 10-hour class on 3/18/21, and to-date there is no documentation of completion.
2. Assistant #2, in the presence of the provider, reported that the training had not been completed.Plan of Correction: Intend to email subsidy effective 7/20/22.
