Comments:
A monitoring inspection was initiated and concluded on 8/03/2022. There were a total of 6 children and 4 staff members present. Children were observed individual play and circle time activity of gross motor skills.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 2 staff records, and 1 household record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4
Description: Based on staff records review, the center did not obtain a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff # 2 (date of employment 6/15/2020) did not have central registry results for Maryland in which the individual has resided in the preceding state within the last 5 years.

Plan of Correction: Will requested staff out of state central registry results for Maryland

Standard #: 8VAC20-800-100-A
Description: Based on child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #3 (date of enrollment 4/01/2022) did not have documentation of physical examination in record to review during the inspection.

Plan of Correction: We let parent know today need physical for child.

Standard #: 8VAC20-800-180-A
Description: Based on staff and household records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #1 Tuberculosis Screening expired on 3/20/2022.
2. Household member #1 Tuberculosis Screening expired on 3/17/2022.

Plan of Correction: Will get updated TB test for staff and household member
