Technical Assistance:
The provider must update her Medication policy or re certify in Medication Administration Training (MAT) as her current certification is expired.

Consultation was provided on obtaining model forms, creating a log for each provider's training hours as well an ensuring the caregivers all have a minimum of 16 hours per year of annual training. Comments:
An unannounced monitoring visit was conducted. 10 children were present with 3 caregivers for 25 points which is within ratios for supervision. The provider was in the home by isolating as she had just returned from a trip. Observed children in 3 groups in the backyard, working on letters and shapes, singing songs and handwashing. Lunch to be served today was chicken soup with rice and vegetables, fruit and milk. No medication was reported to be on-site today. The home has a lot of materials and equipment. Please be sure that each room has space for play and all the hallways are clear of equipment at all times. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-70-A
Description: Based on record review and provider statement, 2 of 2 children's records did not have parent acknowledgement of the Provision of Emergency Relocation form.
Evidence:
Child #1 and Child #2 were missing the signed form that designates the relocation site in case the home needed to relocate in an emergency.

Plan of Correction: The form will be completed by the provider, signed by the parent and placed in each child's file.
