Technical Assistance:
Discussed with the Provider that she should review all FDH standards and ensure her staff are also familiar with the requirements of a licensed FDH.

Comments:
An unannounced renewal inspection was conducted today between 12:25pm and 1:45pm. There were 8 children present (22 points) with the Provider and 2 assistants within the supervision guidelines. The children were observed taking a nap and being held. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-770
Description: Based on observation, a working battery-operated weather band radio was not available to caregivers at all times.

Evidence:
1. The battery-operated weather band radio was not in working order during the time of the inspection.

Plan of Correction: I will get batteries for the radio and make sure it works.

Standard #: 8VAC20-800-800-B
Description: Based on a review of records, the Provider did not review the emergency plan at least annually and document in writing each review.

Evidence:
1. The emergency preparedness plan was last reviewed on 1/27/2019, according to documentation reviewed during the time of the inspection.

Plan of Correction: I will review and correct it by tomorrow.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, 2 out of 4 children's records did not contain the required information.

Evidence:
1. Child #1 was missing documentation of the Emergency Medical authorization and the Emergency Preparedness Plan during the time of the inspection.
2. Child #2 was missing documentation of the Emergency Medical authorization, physical, and immunizations during the time of the inspection.

Plan of Correction: I will ask the parents for this information.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, 3 out of 3 caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

1. Caregiver #1 only had 2 of the 16 required annual training hours documented.
2. Cargiver #2 had 0 of the 16 required annual training hours documented.
3. Caregiver #3 had 0 of the 16 required annual training hours documented.

Plan of Correction: Moving forward, all caregivers will get their 16 hours of training each year with the certificates.

Standard #: 8VAC20-800-510-A
Description: Based on observation, a child was not provided with an individual cot for resting or napping.

Evidence:
1. Upon arrival to the family day home, Child #1 was observed sleeping on the floor with no cot or rest mat.

Plan of Correction: We will make sure all children have their own cot or crib and linens.
