Comments:
A renewal inspection was initiated on 8/9/2022 and concluded on 8/9/2022. There were a total of 10 children ( 28 points) observed with 3 caregivers present. The children were engaged in the following: blocks, indoor climber and outside play. A total of 3 child records and 3 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-490
Description: Based on observation and provider interview, the climbing portions of indoor slides and climbing equipment over 18 inches was over bare floor.
Evidence: The indoor climber equipment which measured approximately 4 1/2 feet tall was over bare floor. From the floor to the platform of the slide measured approximately 2 feet but 2 sides of the climber had wooden rod steps and rock steps that went as high as 4 1/2 feet high. The children, ages 19 months- 2 years were observed climbing on this equipment with a caregiver directly supervising.

Plan of Correction: The children will not be allowed on the climbing equipment until proper surfacing is provided under and around the climber.
