Comments:
An unannounced monitoring visit was conducted today. Two daycare children and two of the provider's children were present with one caregiver for seven points which is within ratio for supervision. Children were observed finishing a snack, free play and watching a show on tv. Home used the living room, bathroom and back bedroom for sleeping (cribs are in the back bedroom). Reviewed physical space, records, training certificates, household member health records and emergency preparation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-750-A
Description: Based on provider statement, topical products have been used on the children but no written authorization is available.
Evidence:
The provider stated that bug spray and diaper ointment are used but no forms have been completed by the parent.

Plan of Correction: Will print a topical authorization for each child in care and have the parent sign the form.

Standard #: 8VAC20-800-830-D
Description: Based on record review, the record for emergency drills was incomplete.
Evidence:
Evacuation drills had not been documented from April 2022-July 2022.

Plan of Correction: Moving forward drills will be written down for each month.

Description: Based on record review, 2 of 2 children's records were incomplete.
Evidence:
Child #1 and Child #2 were missing health insurance policy number.

Plan of Correction: Will request the policy number from both parents to complete the record.

Evidence:
Child #1 was missing the signed acknowledgement of the "Information For Parent" form that provides the parent information on the Family Day Home policies and procedures.

Description: Based on observation, potential poisonous supplies were accessible to children.
Evidence:
Listerine mouthwash and cleaners were on the back of the toilet and not out of reach of children.

Plan of Correction: All products will be moved out of reach in the bathroom.

Standard #: 8VAC20-800-320-B
Description: Based on observation, 2 unused outlets did not have protective covers.
Evidence:
The 2 outlets behind the crib were uncovered.

Plan of Correction: Outlets were replaced during the inspection.

Standard #: 8VAC20-800-690-E
Description: Based on observation, soiled diapers were not disposed of in a foot operated trash can or storage system that prevents access by children.
Evidence:
Soiled diapers were visible in the open trashcan in the bathroom.

Plan of Correction: All soiled diapers will be placed in the diaper genie.
