Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:30 am through 11:30 am. There were a total of eight children (24 points) observed with three caregivers. A total of three children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, games, television time, snack, diapering, hand washing, and other organized activities. Violations:
Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's record (Start date-02/01/22) was missing the parent's employer,employer's address and phone number, a second emergency contact's name, address, and phone number.

Plan of Correction: The provider will get the information from the parents and document the record.

Standard #: 8VAC20-800-680-A
Description: Based on observation, caregivers did not wash their hands with liquid soap and warm running water after diapering a child.
Evidence:
1. Staff #2 was observed changing a child's diaper. After the diaper change, Staff # 2 did not wash their hands with soap and warm running water.

Plan of Correction: Staff will be retrained on handwashing procedures.

Standard #: 8VAC20-800-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering.
Evidence:
1. Staff #2 was observed changing a child's diaper. After the diaper change, Staff #2 placed the child in a crib and did not wash the child's hands.

Plan of Correction: Staff will be retrained on handwashing procedures for children.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use.
Evidence:
1. Staff #2 was observed changing a child's diaper. After the child was changed, Staff #2 did not clean and sanitize the diapering surface.

Plan of Correction: The provider will retrain the staff member about cleaning and sanitizing the diapering surface after each use.
