Comments:

A renewal inspection was conducted on 8/11/2022. There were six children present, totaling 13 points, with the provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 provider/household records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The violations and plans of correction were reviewed with the provider during the exit interview.

Standard #:	8VAC20-800-60-B
Description:	Based upon review of children's records and provider interview, the provider has not ensured that each child's record includes the names, addresses and telephone numbers of two persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1> The record provided for child 4 did not include the street addresses for the two emergency contact persons. 2. The record provided for child 6 id not include the street address for one of the two emergency contact persons. 3. The provider acknowledged the missing street addresses listed above.
Plan of Correction:	The provider responded with the following: All children's records will be checked to ensure that parent's have completed the street addresses for emergency contact persons.
Standard #:	8VAC20-800-240-A
Description:	Based upon observation, the provider has not ensured that areas outside the home are maintained to be safe. Evidence: There are several sprigs of poison ivy growing through the back fence from the neighbor's yard.
Plan of Correction:	The provider responded with the following: The poison ivy will be removed from the fencing. Routine checks will be conducted to ensure that it is not coming through from the neighbor's yard in the future.
