Technical Assistance:
A discussion with the provider included outside procedures- One provider at the front of the line and one provider at the back of line as they go outside.

Comments:
A renewal inspection was conducted on 08/17/2022. There were a total of 8 children (16 points) observed with 2 caregivers present. The children were engaged in the following: playing outside, doing electronics, and washing their hands. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 4 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on a review of child records, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements according to subsection B of this section: 1. Once every six months for children under the age of two years.
Evidence:
Child # 1 (dob 3/28/2018) had immunization records on file dated 2018.

Plan of Correction: I already gave the form to mom. I am waiting for her to bring it back.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas of the family day home were not maintained in a safe condition.
Evidence:
The play house with the blue/teal roof had large cracks on the red shutters.

Plan of Correction: I will remove the play house.
