Technical Assistance:
Consultation provided on children's files and repeat violations.

Comments:
An unannounced renewal inspection was conducted today from 10:05 am to 11 am with the provider, who was present with an assistant and one daycare child. The provider reported that for breakfast the toddler was provided bread, yogurt dip and peaches.
Five children's files and the assistant's file was reviewed. There have been no injuries/accidents or medications on site for any of the children. The provider is up to date with their monthly escape drills. The areas of non compliance in outlined on the violation notice.

Violations:
Standard #: 8VAC20-800-920
Description: Based on observation and interview, it was determined that there was no menu for the current one-week period.

Plan of Correction: I will write and either post or give the menu to the parents at least one week in advance.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's files and interview with the provider, it was determined that there was no physical on file for one child within 30 days of attendance.
Evidence:
Child E enrolled on 7/11/22 and there was no physical on file.

Plan of Correction: I will get the physical.

Standard #: 8VAC20-800-50-A
Description: Based on observation and interview, it was determined that a written record was not maintained of children in attendance each day.
Evidence:
Child E was present in the family day home but was not listed on the attendance. The child had arrived one hour earlier.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-60-A
Description: Based on a review of five children's files and an interview with the provider, it was determined that the children's records were missing information.
Evidence:
1. There were no emergency contacts on file for child A, who enrolled on 8/1/22.
2. There was no physician phone number or address and insurance policy number on file for child B, who enrolled on 7/12/22.
3. There was no insurance policy number or second emergency contact on file for child E, who enrolled on 7/11/22.
All of this information Is required at the time of enrollment.

Plan of Correction: I will get the information from the parents.

Standard #: 8VAC20-800-70-A
Description: Based on a review of children's files and an interview with the provider, it was determine d that the written information provided to parents at the time of enrollment was not complete.
Evidence:
Parents were not provided in writing check in and out procedures for child A, child C, who enrolled on 8/1/22, child D, who enrolled on 7/20/22, and child E.

Plan of Correction: I will include these polices and procedures.

Standard #: 8VAC20-800-690-D
Description: Based on interview, it was determined that the diapering area is not being cleaned and sanitized after each use.
Evidence:
Interviews revealed that the diapering area is not being cleaned with soap and water after each use and the provider is using sanitizing wipes to disinfect the surface after each use. Disinfectant wipes is not an acceptable method to sanitize a diapering area, as the area must be sprayed or dipped in the sanitizing solution and then allowed to air dry .

Plan of Correction: I will include the steps to clean and sanitize the diapering area.
