Technical Assistance:
Discussed safe sleep practices to include no loose bedding, no soft bedding and no stuffed animals and toys in the crib.

Please make sure the children are receiving vigorous outside play daily, weather permitting.

Comments:
An unannounced monitoring inspection was conducted today with the provider, who was present with an assistant and five children, ranging in age from 8 months to 4 years. The inspection took place from 8:20 am to 10:05 am.
The Licensing Inspector was able to observe drop off, free play and breakfast time. Breakfast consisted of peanut butter and jelly sandwich, banana and milk.
The provider's First Aid/CPR is through American Safety Health Institute and expires on 1/9/23. The provider has completed AMAT ( emergency medication administration training) on 3/15/21
No children had medications on site and there have been no injuries/accidents requiring documentation. Five children and 3 caregivers/household members files were reviewed today.
The family cat, Chino rabies vaccination is dated 7/13/22.
The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 22.1-289.036-A
Description: Based on a review of 3 caregiver/ household member's files and interview with the provider, it was determined that two person's did not obtain a repeat background every five years as required.
Evidence:
Person A's central registry background check was dated 5/10/17 and Person Cs central registry background check was dated 5/23/17, neither have obtained repeat background checks within 5 years.

Plan of Correction: We will send off for these background checks.

Standard #: 22.1-289.036-B-2
Description: Based on a review of files and an interview with the provider, it was determined that one person did not have a Fingerprint background check on file within 30 days of turning 18.
Evidence:
Person B, who functions as an assistant and is a household member turned 18 years of age in March 2022 and to date, does not have a fingerprint background check on file.

Plan of Correction: I will have Person B get the fingerprint background check.

Standard #: 8VAC20-800-960-J
Description: Based on observation and interview, it was determined that an infant was fed solid food and the provider did not use a spoon.
Evidence:
The LI observed an 8 month old infant ( Child B) consuming a bottle, an interview revealed that the provider had mixed cereal into the bottle along with the infant's formula.

Plan of Correction: I will no longer add cereal to the infant's bottle and I will make sure solid foods are fed with a spoon, other than finger foods.

Standard #: 8VAC20-800-170-A
Description: Based on a review of files and interview, it was determined that Person B, who turned 18 years of age in March 2022, did not have a tuberculosis test/screening after turning 18 and before coming into contact with children .

Plan of Correction: I will have Person B get the TB test or screening.

Standard #: 8VAC20-800-460-A
Description: Based on observation, it was determined that the family pet was allowed on surfaces where food is prepared or served.
Evidence:
The Licensing Inspector observed, throughout the inspection the family cat on the counters and sink in the kitchenette in the basement. Children's food and drink are stored on that counter.

Plan of Correction: The cat was removed from the daycare area when it was pointed out to me. I will make sure the cat doesn't enter the daycare area.

Standard #: 8VAC20-800-530-F
Description: Based on observation and interview, it was determined that soft bedding and stuffed toys were in and around an infant's crib.
Evidence:
The Licensing Inspector observed a plush Sherpa like blanket hanging on the side of the crib for child A, who is 11 months of age. In the same crib were 3 stuffed toys.

Plan of Correction: The soft bedding was immediately removed when it was brought to my attention. I will make sure that infant cribs remain free of soft bedding or toys.

Standard #: 8VAC20-800-540-B
Description: Based on observation, it was determined that and infant walker was in the daycare area,
Evidence:
The Licensing Inspector (LI) observed an infant walking in the daycare area where two infants and one toddler were present.

Plan of Correction: The walker was removed from the daycare area once it was brought to my attention. I will make sure it remains out of the daycare area.

Standard #: 8VAC20-800-680-B
Description: Based on observation, it was determined that the caregiver did not ensure that children's hands were washed after eating their breakfast.
Evidence:
The LI observed 4 children eating their breakfast and afterward went to play and were not redirected to wash their hands.

Plan of Correction: Once it was brought to my attention, I had the children wash their hands.

Standard #: 8VAC20-800-690-D
Description: Based on observation and interview, it was determined that the diapering surface was not nonabsorbent.
Evidence:
The LI observed an absorbent soft covering on the diapering pad. An interview revealed that the pad is being used with the soft absorbent covering.

Plan of Correction: The absorbent cover was removed once it was brought to my attention and I will make sure diapering is done on a nonabsorbent surface.
