Comments:
A monitoring inspection was initiated and concluded on 8/23/2022. At the time of the inspector's arrival, there were 10 children present, ranging in ages from 2 to 12 years old, with the licensee and one assistant caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 provider / assistant / household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-B-2
Description: Based on a review of records and interview, the licensee did not ensure that each caregiver and each adult household member submits to fingerprinting and submits results of the federal Criminal Background Check for their record.
Evidence:
Four Provider / Assistant / Household Member Records were reviewed.
The record for household member #3, who recently turned 18 years old, did not contain the results of a criminal record check.
Household Member #3 was assisting in the care of children at the time of inspection.

Plan of Correction: The licensee had household member #3 complete the process for scheduling her criminal record check during the inspection.
Household member #3 will provide documentation of the results of the criminal history check to the licensee for her records once they are returned.

Standard #: 8VAC20-800-50-A
Description: Based on observation and interview, the licensee did not always keep a written record of children in attendance each day.
Evidence:
When asked to view the current attendance record, the licensee stated that she has not documented attendance for the current date.
The licensee took out previous completed attendance sheets and added the current date and children's names to an attendance log. She stated that the parents do not always come inside to sign the log when they drop children off in the morning and she has to follow up to record the children's attendance after the parents leave.

Plan of Correction: The licensee completed the attendance record for the children in care on 8/23/2022.
The licensee stated that she will oversee the parents signing children in and out of care at the time that they arrive or depart and will complete the entry if parents fail to do so.

Standard #: 8VAC20-800-170-A
Description: Based on a review of records and interview, the licensee did not ensure that each caregiver and each adult household member has a current report of Tuberculosis screening documenting the absence of tuberculosis in a communicable form.
Evidence:
Four Provider / Assistant / Household Member Records were reviewed.
The record for household member #3, who recently turned 18 years old, did not contain the results of a Tuberculosis screening.
Household Member #3 was assisting in the care of children at the time of inspection.

Plan of Correction: The licensee will request that household member #3 make the soonest available appointment to have a Tuberculosis screening completed and return the documented results within 10 days.
