Technical Assistance:
1. Reviewed emergency drill requirements. Inspector will send drill record form.
2. Discussed documentation for annual training in situations where a training certificate is not provided.
3. Inspector will provide information list for creating a new staff record.

Comments:
An unannounced monitoring inspection was conducted at 12:10pm. Observed 11 children = 30 points with the Provider + Assistant #2. Ratios and supervision were in compliance. Children were napping and reading books, while infants were being held and fed during the inspection. Lunch served today included: chicken, brown rice, corn, grapes, and ham/cheese sandwiches, milk, and water. The daycare areas of the home were well supplied with toys and equipment for the children. Violations:
Standard #: 8VAC20-800-780-B
Description: Based on records reviewed, it was determined that 2 of 2 children?s records did not include documentation that parents had reviewed the emergency contact information for each child and signed the form acknowledging the annual review.

Plan of Correction: The provider will have parents review + sign the form every year.

Standard #: 8VAC20-800-900-B
Description: Based on observations made, it was determined that a child was allowed to lie down while drinking. Evidence: Child #11 ? Was observed lying in a crib while drinking from a bottle.

Plan of Correction: Children will not be allowed to drink while lying down.

Standard #: 8VAC20-800-60-B
Description: Based on records reviewed, it was determined that 2 of 2 children?s records did not include required information.

Evidence:

1. Child #5 ? The parent signed a blank medication policy form.
2. Child #15 ? The parents signed a blank emergency plan form, and did not sign the liability insurance form.

Plan of Correction: The provider said she would complete forms before giving them to parents to sign, and will make sure parents sign all required forms.

Standard #: 8VAC20-800-70-A
Description: Based on records reviewed, it was determined that 2 of 2 children?s records did not include the required information in this regulation section. Evidence: Child #5 + #15 ? Both sets of parents signed a blank ?Information for Parents? form.

Plan of Correction: The provider said she would complete forms before giving them to parents to sign.

Standard #: 8VAC20-800-180-A
Description: Based on records reviewed, it was determined that 3 of 3 caregivers did not obtain a TB screening every 2 years.

Evidence:

1. Provider ? TB test expired 2/7/22.
2. Assistant #1 ? TB test expired 2/5/22.
3. Assistant #2 ? TB test expired 2/7/22.
4. Updated TB tests have not been obtained.

Plan of Correction: TB tests will be completed every 2 years.

Standard #: 8VAC20-800-340-C
Description: Based on observations made and interviews conducted, it was determined that paper towels were not in the bathroom. Evidence: The provider said that children use a shared cloth towel for drying hands.

Plan of Correction: Paper towels to be placed and used in the bathroom.
