An unannounced renewal inspection was conducted on 08/29/2022. There were 3 children in care, ages 21 months ? 3 years, totaling 9 points, 2 caregivers and 1 family member child present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records, 2 caregiver?s records and 4 household member?s records were reviewed. The children were observed during center time and lunchtime. The inspector arrived for the inspection at 10:00am and departed at 12:02pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-110-B Description: Based on review of three children?s records, the provider failed to ensure that all physical records obtained were signed by a physician.
Evidence:
1. Child #1 first date of attendance was 11/03/2020 and on the day of the inspection the physical form that was in the child?s record was not signed by a physician.
2. Child #2 first date of attendance was 12/07/2020 and on the day of the inspection the physical for that was in the child?s record was not signed by a physician.Plan of Correction: Provider will speak with parents and obtain a signed physical record for each child.
