Comments:
An unannounced renewal inspection was conducted on 08/19/2022 from 9:30am to12:00pm. There were 10 children ages 21 months to age 4 years old (23 points) with the Provider and 1 assistant. The programming area, menus, Provider, Assistant and Household member records, 10 child records, attendance, emergency drills, emergency plan and emergency supplies were observed. A new staff member has been hired since the last inspection. Children were observed participating in group play, singing the alphabet, dancing and dramatic play. Staff were current with certification in CPR and First Aid. Staff to child ratios were in compliance during the inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on review of 2 staff records, the provider did not obtain documentation of the results of a national fingerprint background check prior to date of hire.

Evidence: The record for Staff #2 (Date of Hire 8/1/22), did not contain documentation of a fingerprint background check completed prior to their date of hire.

Plan of Correction: The staff #2 background check was done as a household member. Since the inspection, she has had the background check done again and it is on file. I will be sure to have all staff obtain their background check prior to the date of hire.

Standard #: 8VAC20-800-730-D
Description: Based on observation and staff interview, the provider did not ensure that medication was kept in a locked place using a safe locking method that prevents access by children.

Evidence: A medication was stored on the bottom shelf of the unlocked medicine cabinet located in the bathroom that the children use in the programmatic space.

Plan of Correction: The medication has been removed and all medication will be kept in a locked area so that children can not get to it.

Standard #: 8VAC20-800-320-A
Description: Based on observation, the provider did not ensure that small electrical appliances were unplugged when not being used.

Evidence: During an inspection of the kitchen, a toaster, air fryer and coffee maker were all plugged in and not in use.

Plan of Correction: All electrical appliances in the kitchen on the upper level of the home will be unplugged when not in use.

Standard #: 8VAC20-800-510-E
Description: Based on observation and staff interview, the provider did not ensure that rest mats were cleaned and sanitized on all sides weekly or as needed.

Evidence: During an inspection of the programming space, 4 nap mats were observed with tears at the corners and seams preventing them from being sanitized properly. The staff indicated the 4 nap mats were being used for nap time.

Plan of Correction: The mats that were used for play and were torn have been thrown out to ensure that the only mats being used by the children have no tears and can be sanitized and cleaned on all sides weekly or as needed.
