Comments:
A monitoring inspection was conducted on 08/2/92022 between the hours of approximately 2:30 pm and 3:50 pm. There were 10 children present, ranging in ages from 13 months to 4 years, with 2 caregivers supervising. Children were observed during naptime.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 6 staff/ household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-50-A
Description: Based on documentation review, the family day home (fdh) did not keep an accurate written record of children in attendance each day.
Evidence:
1) At the time of entrance, 10 children were in care in the fdh. The attendance record had documented that only 7 children were in care.

Plan of Correction: The provider must ensure to keep an accurate record of daily attendance.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #1 and Child #2 did not contain the policy number of the child's medical insurance.

Plan of Correction: The provider shall contact the parents and request the missing information.

Standard #: 8VAC20-800-210-A
Description: Based on record review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1) On the date of inspection, the records for Staff #1 and Staff #2 did not contain documentation of having completed all of the required sixteen clock hours of annual training during the August 2021 - July 2022 license year: Staff #1 completed 5 training hours and Staff #2 completed 3 training hours.

Plan of Correction: Moving forward, all caregivers must ensure to complete the required annual training hours.

Standard #: 8VAC20-800-320-G
Description: Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) On the date of inspection, a current inspection to the wood burning fireplace of the family day home was not available for review.

Plan of Correction: The provider shall schedule an inspection and provide the inspector with the documentation once completed.
