Comments:
An unannounced monitoring inspection was conducted 8/31/2022 from 10:45 am-12:15pm. There were 9 children (23 points), between the ages of 11 months and 4 years old, with 1 provider for a total of 23 points exceeding the supervision and capacity guidelines for approximately 15 minutes. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 8VAC20-800-800-B
Description: Based on record review and interview, the provider did not review the emergency plan at least annually and update the plan as needed.

Evidence: The emergency plan was last reviewed on 5/19/2020.

Plan of Correction: I will review and update the emergency plan.

Standard #: 8VAC20-800-320-I
Description: Based on observation and interview, there is no fire extinguisher in the family day home.

Evidence: The provider stated that there is no fire extinguisher in the home.

Plan of Correction: I will buy a fire extinguisher.

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, a landline telephone was not available during the family day home?s hours of operation.

Evidence: The provider stated she did not have a landline phone in the home.

Plan of Correction: I will get a landline as soon as possible.

Standard #: 8VAC20-800-570-A
Description: Based on observation, the provider did not ensure that the point system of 16 points per caregiver was followed.

Evidence: During the time of inspection, there were 9 children present, between the ages of 11 months and 4 years, totaling 23 points. There was 1 caregiver present during this time.

Plan of Correction: This will never happen again.
