Comments:
An unannounced monitoring inspected was conducted on August 29, 2022, between the hours of 8:55 am and 10:20 AM. There were 8 children present, with 2 caregivers supervising within the supervision guidelines. The children were observed playing with various age appropriate toys, coloring, finger-painting, and interacting with the caregivers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, fire drill logs, emergencies, and nutrition. A total of 3 child records and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.058
Description: Based on observation, the family day home that serves preschool-age children is not equipped with at least one carbon monoxide detector.
Evidence:
1. There is no carbon monoxide detector installed in the home.

Plan of Correction: I will buy one and get it put up.

Standard #: 8VAC20-800-750-A
Description: Based on record review and interview, the provider did not obtain a written parent authorization as required to use topical skin products such as diaper ointment.
Evidence:
1. No written authorization in the file of Child #1, #2, and #3.
2. The provider acknowledged she did not have authorization forms.

Plan of Correction: I will get parents to sign the form.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit did not contain the minimum supplies.
Evidence: The first aid kit did not have more than one triangular bandage.

Plan of Correction: I will get another triangular bandage.
