Comments:
A subsidy health and safety (SHSI) inspection was initiated on 9/7/22 and concluded on 9/7/22. There were seven children present with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-790-250-F
Description: Based on record review and interview, the provider failed to ensure all staff completed the departments health and safety update course yearly.

Evidence:

1. The record of the provider and assistant were reviewed. No documentation of completing the departments health and safety update course was found for the provider or the assistant.
2. The provider stated she thought they had both completed the course but could not find any documentation in the file or the web page to verify completion.

Plan of Correction: The provider and assistant will complete the departments health and safety update course within a month and will ensure the course is updated yearly from now on.
