An unannounced monitoring inspection was conducted from 12:00 pm - 1:00 pm. There were two children in care being supervised by the provider. The children were observed napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-50-A Description: Based on observation and interviews it was determined that a written record of attendance was not kept daily. Evidence: The daily attendance record was not completed to reflect the two children who were present in the family day home. Plan of Correction: The provider will complete the attendance record daily.
Standard #: 8VAC20-800-60-A Description: Based on interviews with the provider it was determined that the provider failed to maintain an up-to-date record for each child in care. Evidence: The provider reported that there was not a record on file for child A and child B who started on 09/06/2022. Plan of Correction: The provider requested the enrollment records during the inspection from the parents of child A and child B.
Standard #: 8VAC20-800-90-A Description: Based on interviews with the provider it was determined that the provider failed to maintain immunization records for children before the first day. Evidence: Child A and child B (start date: 09/06/2022) did not have documentation of immunization records on file. Plan of Correction: The provider requested immunization records from the parents of child A and child B during the inspection.
Standard #: 8VAC20-800-690-D Description: Based on observation of the diapering surface it was determined that not all diapering requirements were met. Evidence: The diapering surface had a cloth cover on the changing pad making it absorbent and not able to be cleaned and sanitized. Plan of Correction: The provider will remove the cover from the changing pad.
