Comments:
A monitoring inspection was conducted on 09/13/2022 between the hours of approximately 11:30am and 12:45 pm. There were 6 children present, ranging in ages from 15 months to 3 years, with 2 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #4 was dated 03/08/2021 and for Child #5 was dated 08/25/2021.

Plan of Correction: I will give the parents of Child #4 and #5 , ask them to review it and sign it.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The physical examination in the record for Child #1 (start date 09/08/2021) was dated 11/29/2021, and for Child (start date 05/11/2022) was dated 08/08/2022.

Plan of Correction: In the future, I will explain to the parents that they must complete the PE record in a timely manner or else disenroll the child.

Standard #: 8VAC20-800-70-A
Description: Based on record review, parents were not provided with required information form the family day home (fdh) before the child's first day of attendance.
Evidence:
1) The signed acknowledgement as having received the required operating information for the fdh in the children's records reviewed, were dated as follows:
a) For Child #1 (start date 09/08/2021) was dated 10/19/2021.
b) For Child #2 (start date 05/11/2022) was dated 06/29/2022.
c) For Child #3 (start date 11/23/2021) was dated 11/29/2021.

Plan of Correction: In the future, I will make sure the parents are given and sign all required information.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that the child had been adequately immunized before the child attend the family day home.
Evidence:
1) The immunization report in the record for Child #1 (start date 09/08/2021) was dated 05/31/2022, and for Child #3 (start date 11/23/2021) was dated 12/17/2021.

Plan of Correction: In the future, I will ensure the date is correct.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1) On the date of inspection, one electrical outlet in the care area was observed with no protective cover on it.

Plan of Correction: Installed protective cover.
