Comments:
A renewal inspection was conducted today, September 15, 2022. There were 10 children (26 points) present with 3 caregivers. The home, children's records, caregiver records, pet immunization records, cribs, bottles, evacuation drill logs (fire, shelter-in-place and lockdown), first aid kits, emergency preparedness procedures, diaper changing procedures and menu's were reviewed. Standard #: 8VAC20-800-790
Description: Based on observation, not all of the required telephone numbers were posted. Evidence - 1. On the date of inspection, the responsible person for emergency backup care as required in 8VAC20-800 A 3; name and phone number was not posted.

Evidence:
The emergency back-up persons name and phone number was not posted.

Plan of Correction: Information was posted while inspector was on site.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the family day home did not maintain required documents for all children enrolled.

Evidence:
Child #1, #2 and #3's records did not have documentation of names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will request the information from parents and put in each child's records.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, the family day home did not obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Caregiver #1 had documentation of 6 hours of training, Caregiver #2 had documentation of 6 hours of training and Caregiver #3 has documentation of 3 hours of training.

Plan of Correction: All caregiver's will have hours completed by 12/1/2022.
