A monitoring inspection was initiated and concluded on 9/14/2022. There were a total of 2children with 2 staff members present. Children were observed resting.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-100-A Description: Based on children records review , the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence
1. Child #1 (date of enrollment 7/25/2022) did not have documentation of a physical examination in record available for review.Plan of Correction: A physical form was given to the parent for the doctor to complete.
