Technical Assistance:
The provider was not at home during the inspection, the following information was discussed with the substitute provider, diapering mats, cribs' regulations, supervision, activities, records, sheets, cleaning and disinfecting, infant bottles and medications.

Comments:
An unannounced renewal inspection was conducted today from 11:15am-12:55pm. There were 7 children (19 points) directly supervised by 2 staff. The home, 3 staff records, 5 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review, out-of-state child abuse and neglect registry results were not on-file for each staff member.

Evidence:
Staff #3's record does not contain out-of-state abuse and neglect registry results.

Plan of Correction: Out of state record will be requested.

Description: Based on record review, the provider did not ensure that each person in the home becoming 18 years old completes a sworn statement or affirmation.

Evidence:
Household member#3's record does not contain a completed Sworn Disclosure or Affirmation form.

Plan of Correction: Forms will be signed by Household member #3.

Description: Based on observation, diaper ointments provided by parents were not labeled with the children's names.

Evidence:
A tube of Aquaphor and a tube of Zinc ointment provided by parents were not labeled with the children's names.

Plan of Correction: Labels will be provided for all items.

Description: Based on review of emergency supplies, a working flashlight and weather band radio were not kept in a designated area and were available for review during the inspection.

Evidence:
A working battery-operated flashlight and a working battery-operated weather-band radio were not kept in a designated area and were available to caregivers during the inspection.

Plan of Correction: Battery flashlight and radio are currently present.

Description: Based on record review, the provider did not obtain from each adult household member a current Report of Tuberculosis (TB) Screening form documenting the absence of tuberculosis in a communicable form.

Evidence:
Household member#3's record does not contain TB screening or test results.

Description: Based on record review, the provider did not obtain for each household member a current Report of Tuberculosis (TB) Screening form, every two years from the date of the last screening.

Evidence:
Household member #4's record, contains TB screening/test results that were last completed on 2/25/2019.

Description: Based on observation, suffocation hazards were not inaccessible to children under five years of age.

Evidence:
Four inflated latex balloons, which present a suffocation hazard, were observed in the programming area accessible to children under five years of age.

Plan of Correction: Latex balloons which were purchased for birthday parties only, will be removed.

Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age.

Evidence:
A total of 8 electrical outlets were observed without a protective cover in the programming area.

Plan of Correction: All outlets will be covered.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was observed to not be nonabsorbent.

Evidence:
There was a small tear on the surface of the diapering mat, therefore, is no longer considered nonabsorbent.

Plan of Correction: New mat will be purchased.
