Comments:
This monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person inspection was conducted of the program.
The inspection was initiated on September 27, 2022 and concluded September 27, 2022. There were 6 children present, ranging in ages from 9 months to age 9, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 4 child records, I provider and 1 assistant record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-800-780-B
Description: Based on record review and provider interviews, the provider did not ensure to review annually the emergency information with the parent to ensure the information is correct, and obtain the parent?s signed acknowledgment of the review.
Evidence: The provider confirmed the record for Child #3 did not include an updated signature for emergency information as the most recent date was signed was 10-16-2019.

Plan of Correction: Parents was given full VDOE full package

Standard #: 8VAC20-800-790
Description: Based on observation and provider interviews, the provider did not ensure to post all required telephone numbers in a visible area close to the telephone.
Evidence: The provider confirmed they did not have the telephone number posted for the responsible person for emergency backup care.

Plan of Correction: The inspector was informed provider was remodeling the wall.

Standard #: 8VAC20-800-960-E
Description: Based on observation and provider interviews, the provider did not ensue bottles shall be refrigerated and labeled with the child?s full name and the date, if more than one infant is in care.
Evidence: There were two bottles observed on a desk without the infant?s names and dates. Provider confirmed they belonged to two different infants (Child #2 and Child #5).

Plan of Correction: Bottles are labeled and refrigerated.

Standard #: 8VAC20-800-100-A
Description: Based on record review and provider interviews, the provider did not ensue to maintain an up-to-date record at the family day home for each enrolled child with all the required information.
Evidence: The provider confirmed the health examination for child #2 was missing. Child?s first date of attendance was 6-28-2022.

Plan of Correction: Child #2 Mom got full package

Standard #: 8VAC20-800-60-A
Description: Based on record review and provider interviews, the provider did not ensue to
maintain an up-to-date record at the family day home for each enrolled child.
Evidence: The provider confirmed they did not have a record for Child #6, age 9. Provider stated they ?only come for little bit in the afternoon".

Plan of Correction: Child #6 I have her parent's cell phone numbers we're the community I'm helping a mom. Mom on vacation- October 14, 2022 Parents will provide up to date enrollment form.

Standard #: 8VAC20-800-60-B
Description: Based on record review and provider interviews, the provider did not ensue to maintain an up-to-date record at the family day home for each enrolled child with all the required information.
Evidence: The provider confirmed the following was missing from the children?s records as listed below;
1. The record for Child #1 did not include written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.
2. The record for Child #2 did not include the name, address and telephone number of each custodial parent?s place of employment; the name, office address and telephone number of the child?s physician; the emergency contact?s home address; information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation; name and policy number of the child?s medical insurance, names of persons other than the custodial parents who are authorized to pick up the child; chronic physical problems, pertinent developmental information, any special accommodations needed; parent?s signed acknowledgement of the receipt of the information required by to be given to parents prior to child?s first day of attendance.

Plan of Correction: All parents were given VDOE update daycare forms. Parents were given VDOE full package

Standard #: 8VAC20-800-90-A
Description: Based on record review and provider interviews, the provider did not ensue that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.
Evidence: The provider confirmed there was not an immunization record or information for Child #3. Child #3?s first date of attendance was 10-14-2019.

Plan of Correction: Child #3 -Has a complete package Child #3 registration for pre-school mom borrowed files. In the future no parent will have my daycare record.

Standard #: 8VAC20-800-600-B
Description: Based on observation and provider interviews, the provider did not ensue the infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour.
Evidence: The following infants were observed confined to a space for more than 30 minutes.
1.Child #5, 8 months old was observed in a plastic play yard that measured 4 `9?? by 5?2?? from the time inspector entered the home at 3:10 pm and child #1 was still in the space when the inspection ended at 4:30pm.
2. Child #2, 10 months old was observed in a crib from the time inspector entered the home at 3:10 pm until 4:14 pm when child #1 was removed from the crib for diaper change. At 4:19 pm Child #1 was placed back into the crib and was observed still in the crib when the inspection ended at 4:30 pm.
There were 6 children observed in care during the inspection and there was one provider and one assistant. Provider was observed serving snacking and obtaining items requested by the Licensing Inspector (children?s record, staff records and emergency drill documentation). Assistant gave inspector flashlight, radio and first aid kit to review and then was observed ?putting together new children?s packets? as stated by the provider. The assistant was also observed conducting one diaper change.

Plan of Correction: The hours between 3:00-4:45 That's daycare wake-up diaper-changing -feeding and snack-Time. Child #5 was fed changed and enjoying his time watching-himself in the mirror and music, toys-play yard 5'2" x 4'9". We've to accommodate the inspector. The inspector has observed our interactions before with our daycare children. Between 3:00-4:45, The infants and toddlers are in control -providers can not schedule them (children) .Providers had to also assist inspector. Updated package. All parents received 9-27-2022 and 9-30-2022.

Standard #: 8VAC20-800-690-A
Description: Based on observation and provider interviews, the provider did not ensue children?s hands are washed with liquid soap and warm running water as required.
Evidence: Child #2 was observed during a diaper change. Immediately after the diaper change child #2 was placed in a crib and the provider did not wash the child?s hands with liquid soap and warm running water as required after diapering or toileting.

Plan of Correction: Child #2 hands were wash with head -to-toe cleansing clothes with soap wipes.
